This notice also describes our practices related to your PHI and how we may use your PHI within Concord and how under certain circumstances we may disclose it to others outside Concord. This notice also details your rights with regards to your PHI.   • For Treatment: We can use your health information and share with other professionals who are treating you
  • Company Operations: We can use and share your health information to run our business, improve your care, and contact you when necessary
  • For Payment: We can use and share your health information to bill and get payment from health plans and other entities
  • For Research
  • When Required By Law: We can share your health information if required by state or federal law.
  • Respond to Organ and Tissue donation requests: We can share health information with organ procurement originations
  • Medical Examiner or Funeral Director: We can share health information with a coroner, medical examiner, or funeral director to assist them in performing their duties.
  • Health Oversight Organizations: We can share PHI with Health oversight agencies and or personnel that oversee Concord such as the federal agency that oversees Medicare and Medicaid (CMS), and the agency that oversees the clinical laboratory improvement amendments (CLIA) to ensure compliance.
  • Public Safety: We can share your health information to prevent or lessen a serious threat to an individuals or the public health’s safety.
  • Legal Matters: We can use your health information in response to a court or administrative order, or in response to a subpoena.

Your Choices

You have the right to get a copy of your medical record. To do so, please notify us in writing to info@concordsciences.com. You also have the right to ask us to correct and or change your medical record. We may say “no” to your request but will provide an explanation why in writing within 60 days.

Your Rights

In certain cases, you have both the right and the choice to tell us to share information. These include to share with your family, close friends, or other involved in your care, to share information in a disaster relief situation, to share your information in a hospital directory, and to contact you for fundraising purposes. We will never use you information for marketing purposes or sell your information unless you give us written permission.

Changes to this Notice

Concord can change the terms of this notice and the changes will apply to all of your information we have. The updated notice is available upon request, and on our website.
